NHP Plus is a monthly membership with our clinic for adults who do not have health insurance.
You pay a predictable monthly fee and a small flat fee when you are seen. In return you get easier access to your primary care team and help staying ahead of problems before they turn into emergencies.
NHP Plus is not insurance. It does not replace insurance or a health share.

When you’re in need of fast treatment for a condition that is not immediately life-threatening, we’re here to help! You can either come to our doctors’ office in Nacogdoches or see a provider using telemed services. When you can’t come in person, or if it’s the weekend, or if you just prefer to avoid waiting rooms, you can take advantage of our online urgent care triage!
We can quickly transition your care to urgent care facilities or an emergency room if needed.

• Help coordinating care with imaging centers, labs and specialists at cash friendly rates when available
These are services from our clinic only. Hospital care, emergency care and specialist care are separate and billed by those providers.

How Pricing Works
You pay two things:
- Monthly membership fee
- Flat low visit fee of $15 when you are seen
Monthly NHP Plus fee by age:
- Ages 18 to 29: $20 per month
- Ages 30 to 39: $30 per month
- Ages 40 to 49: $40 per month
- Ages 50 to 59: $50 per month
- Ages 60 to 64: $60 per month
Flat visit fee:
- Each office or telemedicine visit: $15 per visit
In house labs, tests, imaging, and procedures are priced separately at cost plus 10%.
Health Share Options (Sedera preferred)
NHP Direct Primary Care + Sedera (Preferred)
Who it’s for
Households who lost ACA subsidies and now face unaffordable Exchange premiums—but still want same/next-day primary care and a credible way to handle big bills.
The offer (simple + honest)
- Direct Primary Care (DPC): $79/month per adult + $10 per visit (charged on the first 3 visits per adult per month, then $0 for additional visits that month). Secure messaging is $0. • Kids are included when at least one adult in the household is enrolled.

Wrap-around (required)
- Preferred: Sedera Health medical cost sharing (you pick an IUA like a deductible: $1,500 / $2,500 / $5,000).
- Alternative: Bring-Your-Own health share (proof required at sign-up and annually).
We do not receive commissions. DPC and health shares are not insurance; sharing follows program rules.
What’s included (DPC scope)
Unlimited primary care access for enrolled adults & their children: in-person & virtual visits, chronic care, preventive care, minor procedures not requiring general anesthesia, secure texting, care coordination, and annual wellness planning.
Pass-throughs at checkout: most labs, imaging, and vaccines (quoted before ordering).
Not included: most prescriptions, specialist/facility charges, and procedures requiring general anesthesia.

- Individual (age 30–39), IUA $1,500 → Sedera $234.53 + DPC $79 = $313.53/mo.
- Individual (age 40–49), IUA $5,000 → Sedera $186.37 + DPC $79 = $265.37/mo.
- Member + Spouse (ages 40–49), IUA $2,500 → Sedera $394.49 + DPC $158 = $552.49/mo.
- Member + Family (2 adults + kids, ages 40–49), IUA $2,500 → Sedera $566.49 + DPC $158 = $724.49/mo.
- Member + Children (age 30–39), IUA $1,500 → Sedera $418.57 + DPC $79 = $497.57/mo.
- Member + Family (2 adults + kids, ages 50–59), IUA $1,500 → Sedera $814.17 + DPC $158 = $972.17/mo (tobacco/vape household: +$75 → $1,047.17/mo).

How Enrollment Works
- Pick wrap-around: Sedera (choose your IUA) or Bring-Your-Own.
- Enroll in NHP DPC: $79/adult; kids included; $10/visit (first 3 per adult/month).
- Upload proof of active health share (re-verified annually; 30-day grace to cure lapses).
- Onboarding visit (virtual or in-clinic), then use NHP first for primary care. Big needs follow your health share’s guidelines.

- NHP Plus Membership is not health insurance
- NHP Plus Membership does not replace health insurance or a medical cost sharing membership such as Sedera
- The membership fee and visit fees pay only for services provided by Nacogdoches Health Partners
- Hospital care, emergency care, specialist visits, surgery, imaging and other outside services are billed separately by those providers
